Client was well dressed with a clean face. Client was given a compliment and he said thank you implementing good manners. Client was awake looking up at the board while his teacher reviewed the lesson of the day. Client was seen in a calm mood, as evidenced by his quiet and relaxed expressions. Client was seen starting his morning assignment and waiting for teacher to give instructions. Client briefly spoke to QMHP. Client stated he felt refreshed and ready to earn a break. Client was reassured he would get his break but he needed to finish his work and prepare for other school activities. (PM)Client expressed good communication verbally and nonverbally and was supported for his cooperation toward peers and authority. Client was able to complete his work and turn it in on time. Client was observed helping his teacher pass out worksheets to the rest of the class. Client was praised and encouraged to keep up the good work. Client showed love and was very productive in class and doing a good job working independently. Client demonstrated good manners in lunch toward peers and staff by being courteous and respectful (e.g. holding the door for others, waiting his turn, and saying please and thank you). Client was monitored in his seat and conversing politely with his peers. Client displayed appropriate social behaviors until it was time to head to psycho therapy. Client presented himself in a pleasant and calm manner going into the group session. Client spoke to his counselor about his day and what he was learning in class. (EOD)Client dislayed good conduct and was cooperative throughout the session. Client displayed good level of participation, completing psycho educational, and making good choices. Client was able to demonstrate a good attitude and work as a group. Client prepared for the next transition, as observed walking safely down the hall back to class. Client returned to his seat to wait for his intervention instructor to pick him up for his tutoring session. Client appeared grateful for receiving extra help in school.
